Meaning

This fatal error message is issued as a result of failing to obtain the first piece metainformation from the FPGA. Since this transmission is the first lifesign the driver expects from the FPGA, it's likely that the FPGA is completely unresponsive. For example, this error can appear if the FPGA has been reconfigured somewhere between the computer's powerup and the host driver's loading into the kernel.

Recommended action

If the FPGA has been reconfigured or otherwise unstable, power off the computer, reconfigure the FPGA, and power it on again.

If this error appears consistently, and there's no reason to suspect the hardware, this error should be reported.
